Member Management Database

Hello,
A while ago I was asked to create a very quick and simple Member Management Database for a friend and this is what I came up with.
Allows you to do the following:
Add a member profile, image, medical information
Add notes to their profile
Send a letter to a single member, certain groups or all members
Export lists of members/groups to CSV
Displays a nice graphical interface for displaying and finding members
Print out member(s) details

I thought I would share it just in case anyone wanted to use it. As I said it is very basic but useful for use as a contacts app maybe.
